Certificate Program Completion Form

Those who are expected to complete the certificate program (15 hours and at
least a "B" grade in each course) need to do as following:

• Use the following link to download the form (a pdf file) and fill it out:
  Graduate Certificate Completion Form
• Bring the completed form to Dr. Lang (207 HEC) before the start of final semester so that he can certify and forward to the College of Sciences for review and approval.
